We Need to Hear from
State and Local Health Departments!

Public health departments provide key immunization services throughout the country. The challenges to health departments to provide vaccine patient education to our increasingly diverse population are becoming more acute. Information technology may be one way to help meet this challenge. If you work at a state or local public health department and have used Healthy Roads Media vaccine information materials with at least two patients then please complete our evaluation survey.

The survey should take only 5-10 minutes to complete. In appreciation for this feedback, we will send you a FREE CD-ROM that contains all the Healthy Roads Media vaccine information. You can duplicate and distribute this CD-ROM for your health department's use.
